NEW PATIENTS: We ask all new patients to arrive atleast 15 minutes prior their scheduled appointment time.  This enables you to complete a detailed medical history while our staff processes your insurance(s) information and prepares your new medical record.

CANCELLATIONS: If you are unable to attend your scheduled appointment, please call us as soon as possible to let us know.  This courtesy is appreciated!
